CPSI, its subsidiary Evident, LLC and their respective subsidiaries and affiliates (collectively “CPSI”) respects your privacy and is committed to protecting any personal information you may provide in the course of using CPSI’s website, www.evident.com. This privacy policy explains how CPSI treats personal information submitted by users of CPSI’s website.

Personal Information Collected:  CPSI collects a limited amount of personal information such as a person’s name, hospital name, mailing address, email address and telephone number.  Personal information is submitted by website users through forms to fulfill requests for information, to create user accounts for the optional User Area and National Association of CPSI Clients (NACC) User Community Website available on CPSI’s website and to identify designated hospital contacts.  CPSI’s website is not intended for use by children and CPSI does not knowingly collect or maintain personal information from individuals under 18 years of age.

Other Information Collected:  CPSI’s website uses “cookies” to provide CPSI with feedback on visitor use of the website and identify areas in which the site may be improved.  Cookies are small data files that are stored on your computer to identify your computer for CPSI’s website.  The cookies used by CPSI’s website do not collect any personal information.

CPSI Use of Personal Information:  Personal information collected on CPSI’s website in response to a request for information is used to fulfill the specific request.  Personal information collected on CPSI’s website in the User Area and the NACC User Community Website is used to maintain user accounts and provide timely information about CPSI products, services and events that may be of interest to the account holders and/or hospital contacts.

Sharing/Updating of Personal Information:  In general, it is CPSI’s policy to not make personal information collected through CPSI’s website available to third parties.  CPSI may, however, disclose personal information collected on CPSI’s website in special cases when we reasonably believe such disclosure would be of value to our customers or is required by law.  Each individual hospital’s contacts identified in the User Area of CPSI’s website are available to CPSI and members of that hospital’s staff who are authorized by the hospital to access the User Area.  Hospital contacts are reviewed and updated by CPSI on a quarterly basis and individuals may notify CPSI of contact changes through the website, by email or by calling CPSI.  The NACC User Community Website is only available to CPSI and CPSI customers who are also members of the NACC.  The NACC User Community Website is intended to provide a forum for discussion, collaboration and networking and, to that end, provides community members with access to, at minimum, the name and hospital of fellow community members.  Community members maintain their account’s user profile and have the option to share additional personal information if they desire to do so.

Security:  CPSI employs appropriate security measures to protect the personal information collected on CPSI’s website.  Such security measures include technical, physical and administrative controls over access to the personal information.  While CPSI takes steps to mitigate security risks, it must be understood that there are security risks inherent to use of the Internet that are outside of CPSI’s ability to control and CPSI cannot guarantee that personal information collected on CPSI’s website will be absolutely safe from intrusion.  Individuals who use CPSI’s website should do their part to protect their personal information by safeguarding passwords and logging off completely when finished using website accounts.

Third Party Links:  CPSI’s website may include links to other third party sites to provide access to additional information; however, this privacy policy does not apply to any third party sites that may be linked to CPSI’s website.  CPSI is not responsible or liable for the content or use of the linked sites.  CPSI recommends that you review the privacy policy of any third party site you may visit via a link on CPSI’s website. 

Privacy Policy Changes:  This privacy policy may be revised from time to time.  When any revisions are made, the revised privacy policy will be posted to CPSI’s website and will be effective as of the date of the last update provided at the bottom of the web page.  As this privacy policy can be updated at any time, CPSI recommends that you review this web page regularly.

Acceptance of the Privacy Policy:  Use of CPSI’s website signifies your acknowledgement and acceptance of this privacy policy.  If you do not agree to the terms of this privacy policy, please do not use CPSI’s website.
